What is the best way to connect an analog fax machine to a FXS port using a PRI feed?

I just wanted to post a fax to email example so everyone can benefit from it. I was looking for it and could not find it, so now that I have implemented it.. I hope you can us it.

I'm trying to enable T.38 Fax in my network environment using UC520. The setting is something like this:

Fax 1 -- UC520 -- CCM -- VOIP -- PSTN -- Fax 2
